5 Healthy Towns Foundation (5HF) is administering an online survey in April.
The foundation is reaching out to residents ages 18+ in the school districts of Stockbridge, Manchester, Grass Lake, Dexter and Chelsea. Those who complete the survey and submit contact information will be eligible for a prize valued at $200 or more. One prize will be awarded in each community.
All responses to the survey are 100-percent confidential.
Data will be analyzed by a third party and only be available in aggregate reports.
