United Methodist Retirement Communities, Inc. (UMRC) announces the establishment of a new option for seniors called Cascade Program for All-inclusive Care for the Elderly, or PACE®.

In collaboration with a partner, Region 2 Area Agency on Aging, Cascade PACE will serve low-income seniors, ages 55 and older, residing in Jackson, Lenawee, and Hillsdale counties, who want to continue to live in their homes but need a place to take advantage of medical and other care services.

An innovative model of comprehensive community-based care, PACE provides an alternative to nursing facility care, allowing eligible participants to live safely and with dignity in their homes and communities.

Integrating Medicare and Medicaid funding, PACE provides a place outside the home to receive acute and primary care, services such as therapy, and all medications and other necessary services, including transportation, for all participants.

Cascade PACE is the first and only PACE center to serve this region, and one of only eight PACE centers statewide. It will begin serving participants in early 2016.

UMRC has recently received a grant of $500,000 from the Edward N. and Della L. Thome Memorial Foundation, which will support start-up costs for Cascade PACE.

“This initial support from the Thome Foundation, known for its commitment to providing for the dignified treatment of older adults, allows this partnership to begin serving seniors sooner,” says UMRC President and CEO John Thorhauer.

“This grant for Cascade PACE helps us bring seniors in Jackson, Lenawee, and Hillsdale counties the medical care and support services they need, while maintaining their independence at home and in their communities.”

Located at 2282 Springport Road in Jackson,  the Cascade PACE Adult Day Health Center will include a medical clinic, therapy and personal care areas, and activity rooms for dining, socialization, and health education.  PACE services include: a comprehensive medical clinic; physical, occupational, and recreational therapy; all pharmacy needs; nutrition and meals; health education; adult day programs, including social and recreational activities; medical specialties such as audiology, dentistry, optometry, podiatry, and speech therapy; and respite care to benefit their primary at-home caregiver.

“PACE’s team of healthcare professionals assesses each PACE participant and develops a person-centered care plan, with customized services to meet each individual’s needs,” explains Thorhauer.

“The PACE team forms a partnership with the participant and caregiver to promote independence and quality of life for the senior,” he said.
